Admissions Coordinator – FULL TIME

Reporting directly to the Director of Admissions, this outgoing and thoughtful individual works to secure patients who require our services by implementing existing marketing strategies, improving on those strategies and helping to identify new strategies.

Position Functions

  • Quickly establish rapport, credibility, and integrity with existing accounts and pending prospects
  • Develop existing relationships into steady referral sources
  • Perpetually research territory for high-return prospects, identify decision makers and get focused face time    with same
  • Effectively articulate our value as it applies uniquely to each prospective referral source
  • Thoughtfully approach everyday and every visit to maximize potential return
  • Maintain (and raise) the values, standards, and culture of this company at all times
  • Reports to ownership and teammates twice a week
  • Hit monthly quota
  • Facilitate/Coordinate all referrals from all current Medical Directors
  • Maintain compliance against various regulatory laws
  • Maintain and Develop business with current Medical Directors
  • Develop marketing strategies for increasing current market share with current Medical Directors
  • Provide feedback from Medical Directors regarding Agency performance
  • Create and propose business development opportunities that will benefit both Medical Directors and our company by increasing visibility in each community.

Education and Experience

  • 2+ years of experience in home healthcare sales
  • Established referral relationships with hospital and physician groups in the area
  • Bachelor’s degree is preferred
  • Knowledge and experience of relevant software applications – spreadsheets, word processing
  • Proficient in spelling, punctuation, grammar and other English language skills

Speech Language Pathologist – PRN

Reporting directly to the Director of Rehab, this compassionate, licensed and technically savvy individual delivers the best possible service by providing therapeutic techniques for the rehabilitation of patients with speech, language, hearing, oral motor, swallowing and cognitive disorders. Also provides coordinated care to patients of all age groups and plans, implements and evaluates patient care plans to restore or maintain patient well-being and collaborates with interdisciplinary and patient care teams.

Position Functions

  • Perform clinical assessment to identify patient needs and appropriateness of patient
  • Document, prepare/revise and complete a Plan of Care (POC), progress notes, phone orders and other clinical record documentation on an ongoing basis
  • Utilize evidence-based pathways to optimize care
  • Analyze patient needs and administer appropriate care as ordered by physician
  • Performs and oversees other team members, when applicable, in the performance of patient care that is consistent with patient needs and desires
  • Applies concepts of infection control and universal precautions in coordinating/performing patient care activities to prevent contamination and transmission of disease
  • Accepts clinical assignments that are consistent with education and competence to care for patients
  • Uses effective interpersonal relations and communication skills; facilitates the use of these skill by other team members to achieve desirable outcomes
  • Meets mandatory continuing education requirements of the agency/licensing board
  • Provides appropriate plan, treatment and evaluation for speech, language, cognitive and swallowing disorders with appropriate tests and measurements. Maintain pertinent records of specific tests performed and progress made
  • Educate patient and family concerning patient deficits, ways to facilitate communication and appropriate discharge plans through family members

Occupational Therapist – PRN

Reporting directly to the Director of Rehab, this compassionate, licensed and technically savvy individual delivers the best possible service by providing coordinated roles and occupational performance of patients with physical, psycho-social and/or cognitive impairments. Provides intervention to deficits and elicits response and collaborates with interdisciplinary and patient care teams.

Position Functions

  • Perform clinical assessment; identifies patient needs and appropriateness of patient
  • Document, prepare/revise and completes a Plan of Care (POC), progress notes, phone orders and other clinical record documentation on an ongoing basis
  • Utilize evidence-based pathways to optimize care
  • Accept clinical assignments that are consistent with education and competence to care for patients
  • Apply concepts of infection control and universal precautions in coordinating and performing patient care activities to prevent contamination and transmission of diseases
  • Assess and facilitate through direct therapeutic intervention patients’ ROM, strength, coordination, need for and use of adaptive equipment and environmental modifications, functional mobility and safety issues as related to patients’ ADL performance needs. Fabricate and maintain adaptive equipment and hand splints
  • Uses effective interpersonal relations and communication skills; facilitate the use of these skills by other team members to achieve desirable outcomes
  • Evaluate patient occupational performance in the areas of self-care, work, leisure and rest.
  • Assess sensory-motor neuromuscular, psycho-social and cognitive-perceptual components of occupational behavior
  • Assess physical, social and cultural components of the patients’ occupational environment

Physical Therapist – PRN

Reporting directly to the Director of Rehab, this registered and technically savvy individual delivers the best possible service by providing coordinated care to patients of all age groups. Plans, implements and evaluates patient care plans to restore and maintain patient well being. Plans, organizes and conducts physical therapy treatment program bases on the medical referral and their evaluation. Also collaborates with interdisciplinary and patient care teams.

Responsibilities:

  • Perform clinical assessment; identify patient needs and appropriateness of patient.
  • Document, prepare and complete a Plan of Care (POC), progress notes, phone orders and other clinical record documentation on an ongoing basis. Revise POC based on evaluation, ongoing assessment data.
  • Performs and oversees other team members, when applicable, in the performance of patient care that is consistent with patient needs and desires.
  • Applies concepts of infection control and universal precautions in coordinating and performing patient care activities to prevent contamination and transmission of disease.
  • Accepts clinical assignments that are consistent with education and competence to care for patients. Plan and provide appropriate treatment for neuromuscular, neurological and orthopedic disorders based on medical referral and therapy assessment.
  • Meet mandatory continuing education requirements of the agency and licensing board.
  • Assist and instruct patient and family in transfer techniques appropriate to patient abilities. Instruct on use of equipment such as walkers, wheelchair, canes, etc. in a safe manner.
  • Utilize appropriate therapy equipment specific to patient diagnosis and needs. Promote Agency philosophy and administrative policies.
  • Demonstrate commitment, professional growth and competency.

Registered Nurse – PRN

Reporting directly to the Director of Nursing, this licensed, compassionate and technically savvy individual delivers the best possible patient care by assessing patient health problems and needs, develop and implement nursing care plans, and maintain medical records.  Administer nursing care to ill, injured, convalescent, or disabled patients.  May advise patients on health maintenance and disease prevention or provide case management.  Licensing or registration is required.  May include advance practice nursing by RNs who have specialized formal, post-basic education and who function in highly autonomous and specialized roles. 

Position Functions

  • Assess Home health patients to identify the physical psychosocial and environmental needs by documentation, clinical record, case conference team report and evaluations
  • Develop, implement, and document the plan of care ensuring quality and continuity of care.
  • Observe infection control measures protecting both patient, and staff (OSHA).
  • Monitor vital signs, diet, physical activity, record and report symptoms and changes in patients’ conditions as appropriate
  • Maintain accurate, detailed reports, records and submit complete reports in a timely fashion
  • Provide direction to the LVN/ HHA to ensure plan of care individually designed for each patient is followed and quality care is delivered
  • Order, interpret, evaluate report/ results of diagnostic tests to identify and assess patients’ condition
  • Monitor all aspects of patient care, modify patient care plans as needed and ensure compliance with requirements of third party payor
  • Collaborate with health care team members to assess, plan, implement, evaluate effectiveness of interventions and modify patient care plans as needed
  • Instruct individuals, families and other groups on topics such as health improvement programs
  • Prepare patients for and assist with examinations tests and treatments
  • Provide skills necessary to perform treatment and procedures according to agency policy in the home setting
  • Administer medications; recommend drugs, medical devices or other forms of treatment, such as physical occupational or speech therapy, inhalation therapy, or related therapeutic procedures
  • Provide effective communication to patient, family, team members and other healthcare professionals
  • Meet mandatory continuing education requirements of the agency/licensing board
  • Demonstrate commitment, professional growth and competency
  • Perform on-call responsibilities and provide on-call service to patients and families as assigned

Qualifications:

  • Bachelors Degree
  • Nursing License for the state of Texas
  • Experience using required equipment like hypodermic needles, nebulizers and accessories, patient care beds and accessories for specialty care, fall prevention devices and accessories, Spiro meters and accessories/supplies
  • 2+ years of experience dealing with disabled or elder care
  • Knowledge and experience of relevant software applications for scheduling and reporting requirements
  • Proficient in spelling, punctuation, grammar and other English language skills
  • Bi-lingual in Spanish a plus
  • Excellent communication skills, both oral and written
  • Compassionate and caring attitude and the ability to deal with people from a wide range of backgrounds, ages and temperament
